Job Summary
Phoenix Home Care and Hospice is hiring compassionate and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide one-on-one private duty nursing for pediatric and young adult clients in their homes. We are currently hiring for Full-Time, Part-Time, and PRN positions with flexible schedules to meet both your needs and the needs of our clients.

Responsibilities
Provide skilled, compassionate, one-on-one nursing care in the home for pediatric and young adult clients
Follow individualized plans of care and deliver treatments as prescribed
Monitor, assess, and document patient condition accurately and timely
Educate and support families to ensure continuity of care
Build meaningful, trusting relationships with clients and families
Collaborate effectively with physicians, care teams, and other healthcare professionals
Requirements
Active and current LPN or RN license
Pediatric or home health experience preferred but not required; full training is provided
Strong commitment to patient-centered care
Excellent communication and interpersonal skills
Ability to pass background checks and comply with all regulatory requirements
